The Family Physician- Care of the Elderly (COE) physician on Seniors Health Outreach Program (SHOP) will complete comprehensive consults on residents living in long term care and Supportive Living facilities in the Calgary Zone.
The COE physician will provide comprehensive geriatric assessments on recent consults and provide follow-up assessments as necessary or as requested by the attending primary care providers.
They will participate in interdisciplinary team conference onsite as required to support staff in the care of the patient. They will also participate in family meetings as required to support families’ understanding of residents’ care needs and to facilitate care planning.
A comprehensive consultation report will be provided to the attending primary care provider.
The successful candidate will also be required to apply for a Clinical Academic Appointment in the Department of Family Medicine, University of Calgary, if teaching learners in Seniors care.

Qualifications:
The ideal candidate will have:
- CFPC and Care of the Elderly designation.
- The candidate must have or be eligible for General Register with the College of Physicians and Surgeons of Alberta.
- This position is not sponsorship eligible.
- Must read, write and speak English.
