Introduction
Participation Lodge Grey Bruce (PLGB) is a social service agency in the Grey Bruce area providing a range of supported living programs and services to individuals with developmental disabilities and acquired brain injury.
These offerings are in the form of supportive care for people living in our main group facility (the Lodge); and for those with greater independence residing in their own home or apartment in the surrounding communities we serve.
Founded in 1974, the organization was the beneficiary early on of the donation of a 25-acre property by LutheRanch, a non-profit charitable corporation of the Lutheran Church. The physical building known as the Lodge opened its doors on this property in 1982 and the agency has grown considerably in scope since.
PLGB provides Assisted Living Services and/or Adult Developmental Supports and Services to individuals who live at the Lodge and residing in community settings. The organization operates under the auspices of and is funded by the Ontario Ministry of Health (OH) and the Ontario Ministry of Children, Community and Social Services (MCCSS).

Current and Future States
PLGB is at an exciting and key juncture in its history. Our Agency is in transition from an institutional-type long-term care model to a person-centred model of supports and services, promoting greater choices for people and assisting them to develop and maintain independence. This transition is aligned to MCCSS’ strategic direction, a multi-year plan to enable people with developmental disabilities to live their best lives.
It is also in keeping with OH policy and funding for PLGB as a developmental services agency, not a long-term care operation with a medical model of care.
The significance of this organization change cannot be over-stated. It involves a rethink and transformation of all aspects of our operations and approach to people we support; the development of external partnerships with medical and allied health providers and other key stakeholders; and establishing a new set of key performance indicators, measurement and reporting systems aligned to a new person-directed model of support.
PLGB is in the process of developing a new strategic plan to address these imperatives, leverage our strengths, identify gaps in capability, and define priorities going forward. This is an exciting time to join our agency!
Job Summary The Support Worker provides essential daily living supports to individuals. The incumbent may be designated to work at of one or more PLGB Programs. Staff are on-site 24/7 to assist with scheduled and unscheduled support such as personal care, community integration, medication support, meal preparation, and daily routines. This program supports clients’ independence and also fosters a sense of belonging and connection within the community.
Key Responsibilities
HEALTH AND WELLNESS
- Observes and implements appropriate actions to promote healthy living, prevent illness and accidents
- Assists individuals in scheduling,
keeping and following through on all health appointments
- Physical Development and Health
- Promotes independence as outlined by the client
- Behavioural management
- Assist individuals with household management (i.e. meal preparation, laundry, light housekeeping, cleaning, decorating, general tidying of client’s apartments/living spaces)
PERSONAL CARE INCLUDING BUT NOT LIMITED TO:
- Oral hygiene
- Hair care
- Washing/Bathing
- Menstrual care
- Transferring/positioning/turning
- Dressing/undressing
- Assistance with eating, oral and tube
- Medication observation and/or administration
- Range of motion
ADVOCACY, COMMUNITY/SERVICE NETWORKING & COMMUNICATION:
- Staff are knowledgeable about the formal and informal supports available in their community and are skilled in assisting the individual to identify and gain access to such support
- Staff support the individual in development of friendship and other relationships
- Enhance the ability of individuals to lead a self-determining life
- Assists to coordinate necessary and voluntary community outings
- Assisting with communications
- Provides individuals with opportunities and experiences provided to others in society
- Encourage client regarding the realization of their hopes, dreams, choices and rights
- Respond to client concerns in a timely and respectful manner and escalate formal complaints through the organization's established complaints process.
- Effective sensitive communication skills to build rapport and channels of communication by recognizing and adapting to the range of individual communication styles
- Maintains accurate records collecting, compiling and evaluating data, and submitting records to appropriate sources in a timely manner
- Maintain standards of confidentiality and ethical practice
CRISIS INTERVENTION/HEALTH AND SAFETY:
- Understand and follow health and safety policies and procedures
- Always demonstrate secure work practices
- Report identified items that need to be repaired
- Reports on all unsafe conditions, practices and equipment
- Reports on all accidents and completes incident reports
- Informed on expectations of crisis prevention, intervention and resolution techniques
- Practice appropriate first aid/safety procedures when responding to emergencies
Qualifications Education & Training
- Relevant education in social services, healthcare or high school diploma with relevant experience
Skills & Experience
- Experience working with people with physical and developmental disabilities preferred
- Must have CPR and First Aid or be willing to obtain
- Must have non-violent crisis intervention training or be willing to obtain
- Vulnerable Sector Screening
- Full G driver's license

- Strong written, verbal and listening skills
- Experience working with persons with physical and developmental disabilities preferred
- Excellent judgment in dealing with sensitive and confidential information in a diplomatic, professional and tactful manners
- Maintains standards of confidentiality and ethical practice according to PLGB Code of Conduct
- Self-directed with the ability to work independently and as a team member
- Ability to work professionally, effectively and co-operatively with all staff to build relationships
- Demonstrated computer skills in Microsoft Office Suite and other software as used within agency practices
- Strong organizational skills with ability to respond to multiple tasks and competing priorities

Physical Requirements
- Intermittent periods of sitting/standing and computer/phone use
- Ability to walk, stand, kneel, crouch, stoop, and reach above head

- Ability to support adult body weights in activities of standing, walking, vehicle entry
