We are seeking a dedicated and compassionate Temporary Full-Time Unregulated Care Provider/Registered Practical Nurse (UCP/RPN) to support our healthcare team during a maternity leave coverage period. The successful candidate will play a vital role in delivering high-quality patient care across various settings, including nursing homes, hospitals, and community-based environments. Duties
- Provide comprehensive patient care in accordance with established protocols, including medication administration and vital sign monitoring.
- Assist with activities of daily living such as bathing, dressing, mobility support, and nutritional assistance.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to patient needs.
- Support patients with dementia, seniors, pediatrics, or those receiving hospice or palliative care by applying specialized approaches.
- Document patient information accurately using electronic health record systems such as Epic or Cerner.
- Perform phlebotomy procedures and laboratory tests as required to support diagnostic processes.
- Ensure compliance with infection control standards and safety regulations within all care environments.
- Engage in case management activities to coordinate services and facilitate smooth transitions across care settings.
Skills
- Proven experience working in nursing homes, hospitals, or acute care environments with a focus on patient-centered care.
- Solid knowledge of medical terminology, medication administration protocols, and clinical procedures.
- Experience in dementia care, senior care, hospice services, or pediatric settings is highly desirable.
- Familiarity with home care services and assisted living environments enhances the ability to adapt to various patient needs.
- Competence in laboratory procedures such as phlebotomy and laboratory testing processes.
- Proficiency with electronic health record systems like Epic or Cerner for documentation and communication.
- Excellent communication skills combined with compassionate patient interaction and teamwork abilities.
- Ability to manage multiple priorities efficiently while maintaining high standards of safety and quality of care.
