Bayshore Health Care is seeking an Occupational Therapist to join our Integrated Care Solutions team in Tillsonburg, Ontario. You will assess, treat, educate, and manage care for older adults transitioning from hospital to home, within an integrated team and shared client record.
As part of Bayshore ICS, you will collaborate with Nurses, PTs, social workers and support staff to deliver evidence-based OT services, ensuring protected, patient-centered outcomes in community settings across Ontario.
