You will engage in comprehensive primary care, emphasizing health promotion and chronic illness management, in collaboration with Nurse Practitioners and other healthcare professionals. Ensure cultural safety and client-centered care while supporting community health initiatives.
Key Responsibilities:
• Provide thorough primary care services to clients
• Conduct patient assessments and manage care plans
• Engage with interdisciplinary teams during case reviews




• Maintain precise documentation in line with standards
• Practice with cultural awareness and client-centered approaches
Requirements:
• MD and CCFP certification necessary
• License for Independent Practice with CPSO preferred
• Experience in community health or with Indigenous populations
