Detailed Overview

The Community Health Nurse (CHN) – Licensed Practical Nurse works independently in the community setting, collaborating with an interprofessional team to manage an assigned client caseload. Responsibilities include assessments, coaching, interventions, client care services, and follow‑up to enable clients to live confidently at home and/or community. The role emphasizes promotion, maintenance, and restoration of health, particularly in treating chronic diseases through teaching, counselling, and direct client care. The CHN facilitates client transitions across the healthcare continuum, ensuring linkages with acute, primary, and community care providers.

Responsibilities

- Establish therapeutic relationships with clients using interpersonal and interviewing techniques, in person or over the phone,



ensuring client choice and autonomy.
- Screen referrals, conduct individualized client assessments, create interprofessional care plans and interventions, and assist clients in achieving optimal function by facilitating timely health services.
- Develop comprehensive shared client care plans and coordinate transition to referring sources, primary/community care providers, or community agencies.
- Provide direct client care and identify required services per guidelines, policies, and evidence‑based best practice.
- If required, assign direct client care tasks to Community Health Workers, providing training and supervising to ensure competency.
- Facilitate care conferences with the interprofessional team and primary care provider to review plans and determine referral timing, evaluate effectiveness, and develop next steps.
- Discuss clinical findings with client/family, develop action plans and set goals for self‑management, self‑care, and improved quality of life.
- Support clients and families before, during, and after conferences and clinic visits, advocating for client choices and providing direct care during transition to services.
- Arrange and participate in joint home visits with professionals, and advise clients/caregivers on community resources.
- Provide health counseling, including education and wellness promotion through clinic, telephone, or home visits.
- Maintain clinical records (intake screens, assessments, interventions, treatment formulations, care plans, progress notes) and statistical information per policies.
- Maintain skilled practice growth, stay current with literature, workshops, and networking.




- Participate in department quality improvement, risk management, and research opportunities.
- Orient and educate nursing staff and students, acting as preceptor as appropriate.
- Contribute to development and revision of standards of care, policies, and procedures, advocating for improvement.
- Perform other related duties as assigned.

Qualifications

Education and Experience

- Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- One year of recent clinical experience assessing and treating complex geriatric and/or adult patients with chronic health conditions in an acute or community/outpatient setting, including recent experience in care and discharge planning, or an equivalent combination of education, training and experience.
- Valid BC driver’s license and access to a personal vehicle for business‑related purposes.

Skills and Abilities

- Demonstrated knowledge and competence in gerontology and caring for adults with complex frailty and chronic illnesses.
- Proficiency in completing initial and ongoing client assessments and providing nursing care through therapeutic interventions.
- Strong communication skills, both verbal and written.
- Ability to integrate and evaluate pertinent data from multiple sources to problem‑solve effectively.
- Knowledge of broad health‑care services, community resources, and agencies within the continuum of care.
- Ability to promote client‑focused care with sensitivity to diverse cultures and preferences.
- Capacity to independently manage and prioritize diverse healthcare issues.
- Skill to teach clients about health promotion, self‑management, and care principles.
- Ability to work effectively in a dynamic environment with changing priorities and as part of an interprofessional team.
- Familiarity with related equipment and software applications.
- Physical ability to perform the duties of the position.
