Overview

This is a permanent part‑time physician position (not locum) within the Vancouver Division of Family Practice to augment a family practice with a long‑term care (LTC) panel. The role involves caring for frail elderly patients over a long‑term period (>8 weeks) at Adanac Park Lodge, a 73‑bed LTC home.

Responsibilities

- Provide complex, holistic, resident‑centered care in partnership with an interdisciplinary team (physicians, NPs, nurses, allied health, and leadership).
- Conduct regular proactive visits, on‑site medication reviews, and attendance at care conferences.
- Participate in the 24/7 on‑call rota and complete all required documentation in charts.




- Serve in mentorship and onboarding for new LTCI practitioners.
- Engage in LTC‑specific events, projects, and after‑hours care program.

Qualifications

- Licensed physician with experience or willingness to develop expertise in long‑term care.
- Specialized mental‑health skills are an asset.
- Must hold or be willing to obtain Vancouver Coastal Health (VCH) privileges and credentials for the care home.
- Solid communication skills and a commitment to best‑practice expectations of LTCI.
