Job Summary
We currently have an exciting opportunity for a Social Worker, Long Term Care to provide services at Bradley Centre – Chilliwack General Hospital located in Chilliwack. This role is part of an interdisciplinary team focused on promoting self‑determination and delivering high‑quality care and support to individuals and families.
Requirements
- Bachelor’s degree in social work
- One year’s recent, related experience in a hospital setting or an equivalent combination of education, training, and experience
- Full registration with the BC College of Social Workers
Detailed Overview
Reporting administratively to the Manager, Clinical Services, and clinically to the Clinical Practice Leader, the Social Worker, Long Term Care uses professional and program standards in providing psycho‑social assessment, crisis intervention counselling, and specific resident treatment, while assisting with tasks shared among all team members.
Responsibilities
- Facilitate admissions by liaising with Access, Home Health, and clients; conduct admission interviews and tours; issue medical forms; complete required documentation; schedule admissions and hold admission conferences for staff.
- Assist residents and family members/significant others with financial matters including equipment acquisition, government applications, and other relevant matters; liaise with agencies and interdisciplinary team members.
- Provide counseling, education, and support to residents and family members/significant others from pre‑admission to discharge or death.
- Liaise with interdisciplinary team members and staff; arrange and chair care‑plan conferences.
- Maintain records, including statistics, and document social work services, assessments, and interventions in interdisciplinary focus notes.
- Act as a resident advocate, maintain confidentiality, and practice in an ethical manner; consult with the Professional Practice Chief and other Social Workers on professional practice issues as required.
- Coordinate and chair Resident Council and Family Support Group meetings; take minutes and arrange speakers.
- Participate in the development, implementation, and evaluation of skilled standards and practices by providing input to the Professional Practice Chief, Social Worker, and via Professional Practice Committee activities.
- Participate in continuous quality improvement activities, OH&S; and related safety programs, accreditation, and regional initiatives as directed by the Manager.
- Perform other related duties as assigned.
Education and Experience
Baccalaureate degree from an approved school of Social Work; one year’s recent related experience working with the elderly in residential settings.
Skills and Abilities
- Ability to work independently and contribute to and function within an interdisciplinary team.
- Ability to plan and organize work and set priorities.
- Effective communication skills, both verbal and written.
- Ability to establish and maintain positive relationships with residents, family members/significant others, staff, and volunteers.
- Operational competency with related equipment.
- Physical ability to carry out the duties of the position.
