Overview

In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, this role provides and coordinates pre‑ and post‑operative nursing care including assisting with surgical and interventional procedures and discharge teaching for adults and children in an inpatient unit and/or outpatient clinic or setting. This role works collaboratively with an inter‑professional care team to deliver person‑centered care.

Responsibilities

- Assesses patients and collects information from a variety of sources using observation, communication, data review, and physical assessment. Makes nursing diagnoses and determines appropriate nursing interventions. Plans, performs, and evaluates nursing care.
- Observes patient acuity for significant changes and intervenes to provide and manage necessary care. Resolves problems or emergency situations by planning and performing nursing interventions and evaluating outcomes.
- Assists physicians with therapeutic interventions by positioning patients and using procedure‑related medical equipment. Assembles, maintains, and is responsible for all procedural equipment.
- Collaborates with patients, families/ caregivers to develop the care plan and encourages family participation in its implementation and ongoing modification. Establishes a therapeutic relationship by demonstrating empathy,



trust and respect, advocating for the patient’s rights to privacy, dignity, and access to information, and ensuring the patient’s choice and autonomy in decision‑making and care planning.
- Participates in care and discharge planning on admission: ensuring the care plan is established and implemented, updating the plan and intervening as appropriate; collaborates with the inter‑professional care team to ensure comprehensive care planning and delivery; and coordinates with the Patient Care Coordinator on issues affecting the care/discharge plan.
- Coordinates nursing care, sets priorities, assigns and delegates tasks to support staff, and makes referrals to the inter‑professional care team as per protocols and practice standards.
- Collaborates with members of the inter‑qualified care team — physicians, licensed practical nurses, patient care coordinators, clinical nurse educators, and other health care professionals — to identify and resolve issues, adjust care plans, and address changes in patient condition.
- Teaches patients and families/caregivers about illness prevention, health restoration and health maintenance to help patients achieve their health care goals. Assesses learning needs and provides information and demonstrations of care activities to support self‑care and the discharge plan.
- Plans and discusses referral information with patients and their families/caregivers and, in collaboration with the inter‑professional care team, initiates referrals to appropriate community agencies.
- Documents assessments, nursing diagnoses, observations, interventions, patient/family responses, outcomes, and referrals according to practice standards and Fraser Health procedures.
- Participates in nursing committees, care conferences, and councils as assigned or per agreement with the Manager.
- Participates in research and quality improvement activities, identifying patient care issues, collecting data, and recommending corrective actions and changes to practices, procedures or protocols.




- Participates in and attends in‑services and other educational programs; identifies learning goals and maintains updated clinical competence and knowledge within the area of practice.
- Acts as a preceptor or mentor to assigned new nursing staff and students; shares information, demonstrates care activities, and participates in orientation.
- Provides feedback to the Manager/Delegate regarding the effectiveness of patient‑care programs, policy/clinical decision‑support tools (e.g., clinical pathways, standards, order sets, procedures), and equipment.
- Performs other related duties as required.

Education and Experience

Graduation from an approved School of Nursing. Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).

Two (2) years of recent relevant clinical experience in the designated clinical area and completion of postgraduate coursework and/or certification applicable to the designated clinical area, or an equivalent combination of education, training and experience.

Skills and Abilities

- Ability to initiate and maintain intravenous therapy.
- Ability to complete initial and ongoing patient assessment (clinical and diagnostic reasoning) and provide nursing care through appropriate/prescribed technical, therapeutic, safety‑type interventions.
- Ability to communicate effectively with patients, families/caregivers, the public, medical staff, and members of the inter‑professional care team using verbal, written, and electronic communication means.
- Ability to promote person‑centered care that demonstrates empathy for patients and families/caregivers, sensitivity to diverse cultures and preferences, and advocate for patient rights.
- Ability to integrate and evaluate data from multiple sources to problem‑solve effectively.
- Ability to teach and instruct patients and families/caregivers about essential topics for their health care and well‑being.
- Ability to organize and prioritize work.
- Ability to collaborate with and promote cooperation among members of the inter‑professional care team.
- Ability to use factual information, prior learning, and basic principles and procedures to support nursing decisions and actions with relevant evidence‑based or evidence‑informed practice.
