Hours of Work : Monday - Friday; 8:45/11:45am -5:00/8:00pm

These hours are dependent on the operational requirements of the organization

Mount Carmel Clinic operates out of multiple locations; you may be required to work in other areas

Position Summary
The Primary Health Nurse II provides health education and care within the scope of nursing practice. As a member of a client-centered interdisciplinary team, the Primary Health Nurse II provides care to individuals promoting wellness, prevention of disease and/or disability and the management of chronic diseases/conditions. The incumbent works as an advocate on behalf of the client to ensure the appropriate links/referrals to other human services that would enable the client to achieve their health goals.

Qualifications
Education

Graduate of approved Registered Nursing education program, BN preferred.

Must be a member in good standing with CRNM.

Skills, Expertise And Experience

A minimum of five (5) years’ experience working as a Registered Nurse.

A minimum of three (3)



years’ experience working in a community health setting.

Knowledge and Experience working with Students and/or Volunteers.

Current CPR Certification at Basic Rescuer Level.

Experience working with service specific programs including EMR, eChart, and Accuro.
