Responsibilities
Assess clients’ needs through interviews, observations, and documentation to develop personalized care plans.
Provide emotional support and counseling to individuals and families experiencing crisis or hardship.
Connect clients with community resources such as housing, healthcare, employment, and educational services.
Advocate on behalf of clients to ensure they receive appropriate services and support from external agencies.
Monitor client progress regularly, adjusting intervention strategies as necessary to promote positive outcomes.
Maintain accurate case records, documentation, and confidentiality in compliance with organizational policies.
Collaborate with multidisciplinary teams including healthcare providers, educators, and legal specialists to coordinate comprehensive care.
Requirements
Bachelor’s degree in Social Work, Psychology, Sociology or a related field; Master’s degree preferred.
Valid licensure or certification as required by local regulations for practicing social work.
Proven experience working with diverse populations in community or clinical settings.
Excellent communication skills with the ability to listen actively and convey information clearly.
Strong organizational skills with attention to detail for case management and documentation.
Ability to handle sensitive situations with empathy, professionalism, and cultural competence.
Knowledge of local social services resources and referral processes is highly desirable.
