Older Adult Outreach Nurse – Powell River, BC
Salary Range: CAD $49.20/Hr. – $63.47/Hr.

Key Responsibilities

Work with older adults experiencing dementia or major psychiatric disorders, concurrent disorders, and other issues associated with aging.

Conduct psychogeriatric assessments, crisis intervention, case coordination, and treatment planning.

Provide consultation/liaison work with general practitioners, caregivers, family members, and staff in continuing and residential care as well as hospitals.

Coordinate community resources and deliver educational programs.

Maintain relationships with clients, families and colleagues to provide appropriate treatment services based on best practices.

Share knowledge and expertise with colleagues through consultations, in‑services and presentations.

Act as a clinical resource for nurses and other health care providers in the hospital and community.

Provide direct care and outreach services as needed.

Education & Experience

Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).

Three (3) years of recent related experience working in geriatric mental health and addictions or an equivalent combination of education and experience.





A valid B.C. Driver’s License and use of a vehicle.

Knowledge, Skills & Abilities

Knowledge of gerontology and geriatric psychiatry, and nursing standards of the Canadian Gerontological Nursing Association.

Knowledge of concurrent disorders, assessment and treatment of psychiatric clients including geriatric psychopathology.

Knowledge of issues associated with aging, chronic illness, psychosocial factors such as caregiver issues, family dynamics and communication patterns.

Knowledge of detoxification/withdrawal management, abstinence/harm reduction models for geriatric patients with substance use disorders.

Knowledge of psychopharmacology (indications and side‑effects).

Knowledge of crisis intervention and psychiatric services/community resources.

Demonstrated ability to provide counseling, psycho‑education and consultation services.

Ability to act as a liaison between hospital and community geriatric services.

Knowledge of family dynamics, end‑of‑life issues, and system theories.

Ability to perform relevant assessments using validated tools.

Develop holistic care plans and build rapport, trust and ethical relationships with clients and families.

Effective communication orally and in writing with clients, families, physicians and other health care staff.

Work independently and in a consultative/collaborative manner as a team member, leader and facilitator.

Promote client‑focused care demonstrating sensitivity to diverse cultures and social justice concerns.





Knowledge of legislation and policies related to client care, including abuse and neglect legislation.

Adaptability to new or unexpected events and conflict situations.

Application of theory and practice to case management and effective conflict resolution.

Develop and deliver educational programs and materials for staff, patients and families.

Basic research knowledge, including ethics and data analysis methods.

Planning, organization, scheduling and prioritization of work.

CPR certification.

Proficiency in nursing equipment and related computer applications.

Demonstrated physical ability to perform duties of the position.

Work Hours
Hours of work, including days off and work area, may be subject to change consistent with operational requirements and the Collective Agreement and applicable statutes.
