In this entry-level role, you will work independently and as part of a team to manage client caseloads while providing assessments, coaching, and interventions. Your efforts will contribute to the quality of life for patients in the community. Key Responsibilities:
Screen referrals and perform client health assessments
Develop interprofessional care plans for complex conditions
Provide direct care and client education in various settings
Coordinate joint home visits with other healthcare specialists
Maintain clinical records in line with policy Requirements:
Registered Nurse certification with BCCNM
One year of recent experience in community/outpatient care
Access to a vehicle for client visits
Demonstrated competence in community health
