You will work collaboratively, developing personalized care plans while engaging with clients' physical, cognitive, social, emotional, and spiritual needs. This role allows for qualified autonomy while improving health outcomes for clients through supportive teamwork and community connections. Key Responsibilities:
Provide comprehensive care as part of an interdisciplinary team
Act as the primary contact for assigned clients
Conduct assessments and create interdisciplinary care plans
Facilitate communication among interdisciplinary team members
Support clients through care transitions and community connections Requirements:
Registered in good standing with the College of Nurses of Ontario
Two years of nursing experience required
Experience in home/community care or related fields
Solid assessment and clinical leadership skills
