In this role, you will support the Emergency Department at VGH, ensuring integrated care through collaboration with interdisciplinary teams. Your commitment will center around facilitating protected discharges and enhancing patient outcomes through community resources.

Key Responsibilities:
• Formulate and implement patient discharge plans
• Provide care that centers on patient and family needs




• Reduce admissions for complex geriatric patients
• Collaborate with diverse health professionals
• Conduct sensitive Goals of Care discussions with families

Requirements:
• Full CPSBC License is required
• CMPA membership mandatory
• CCFP certification is essential
• Interest in geriatrics is valued
• Strong interpersonal and communication skills needed
