About The Role

We are currently recruiting for a full time Care Partner based in Pakenham, supporting clients across the Southern Region (Casey, Cardinia, Bayside) and Gippsland.

person‑centred care to clients in their homes and communities
Conduct comprehensive assessments, develop care plans and coordinate services
Empower clients and carers to make informed choices about their care
Ensure compliance with aged care quality standards and organisational policies
Maintain accurate case notes, documentation and client confidentiality
Work collaboratively within a supportive, multidisciplinary team and elevate concerns where required
