About Crosstown Clinic
Crosstown Clinic is a harm reduction-driven team offering medical‑grade heroin (diacetylmorphine) and hydromorphone within a supervised clinical setting to chronic substance‑use patients, supported by an on‑site pharmacy and an interdisciplinary care team composed of doctors, nurses, social workers and substance‑use disorder counsellors. Life‑skills counselling, housing referrals and legal assistance are also provided.
Responsibilities
Client Care & Monitoring – Provide compassionate, evidence‑based nursing care to clients with substance use disorders within a harm reduction framework. Monitor client health and wellbeing before, during, and after treatment, respond to medical or behavioural crises, and collaborate with the interdisciplinary team to ensure safe and effective care delivery.
Care Planning & Documentation – Assess, plan, implement, and evaluate individualized care plans using clinical judgment and best practices. Maintain accurate and timely documentation of assessments, interventions, progress notes, and discharge plans in accordance with professional standards.
Education & Advocacy – Educate and coach clients and families on health‑promoting behaviours, harm reduction strategies, and treatment options. Advocate for clients’ dignity,
rights, and access to care, ensuring services are provided with compassion and respect.
Collaboration & Leadership – Work closely with physicians, social workers, counsellors, and other team members to coordinate care and facilitate continuity across services. Provide clinical guidance to support staff and contribute to quality improvement, research, and educational initiatives within the clinic.
Education and Credentials
Registered Nurse (RN) or Registered Psychiatric Nurse (RPN) with registration in British Columbia.
Skills and Abilities
Effective verbal and written communication.
Knowledge of and ability to perform CPR appropriate for the area of service.
Team collaboration within an interdisciplinary setting.
Decision making and analytical skills.
Conflict resolution.
Physical ability to carry out the duties of the position.
Prioritization and organization of work.
Operation of related equipment.
Teaching and identifying patient learning needs.
