Responsibilities Provide assessment consultation and education to patients, family members and the interprofessional team within the acute and subacute adult patient population at Oak Valley Health – Markham site Reactivation Care Centre.
Utilize a goal‑oriented, client‑centered approach to care.
Participate in ongoing program development and quality improvement activities.
Perform frequent moderate and heavy exertion: lift over 60 lbs., manage fast pace and manual patient handling including manual chest care, repositioning, transferring, lifting and ambulation activities.
Qualifications Current registration with the College of Physiotherapists of Ontario.
Minimum 2 years experience in an acute care and/or subacute care setting.
BLS certification is required.
Previous experience with surgical, rehabilitation and emergency patient populations.
Ability to work effectively and efficiently in a fast‑paced environment.
Demonstrated critical thinking skills in a subacute care workplace.




Excellent discharge planning skills with knowledge of community resources.
Excellent communication and interpersonal skills.
Ability to collaborate with members of the interprofessional team, including support staff.
Good computer skills and experience with electronic documentation preferred.
Evidence of continuing education and ongoing professional development.
Demonstrated good attendance and performance records.
Commitment to a healthy and safe workplace and compliance with hospital policies, health and safety legislation and best practices.
Work Schedule Variable schedule with a focus on Monday/Tuesday, some weekly variation and one weekend included. Shifts 10am–6pm, subject to operational changes.
Compensation OPCOM12 range: $41.36 – $57.27 per hour.
