Spectrum Health Care is expanding its Allied Health team and is looking for a full-time Occupational Therapist focused on integrated care transitions. This position is pivotal in supporting patients from hospital discharge to home rehabilitation.

As an Occupational Therapist, you will engage in comprehensive in-home assessments and collaborate with patients and families to create bespoke care plans. Partnering with multidisciplinary teams, you’ll play a key role in evaluating patient progress and implementing adaptive strategies to enhance daily living activities. This critical role aims to improve patient outcomes and reduce readmissions.

Key Responsibilities:
• Assess and support patients’ functional independence
• Conduct detailed in-home assessments for clients




• Develop team-oriented care plans with family input
• Work closely with an interdisciplinary team
• Provide training on adaptive and safety equipment

Requirements:
• Completion of a recognized Occupational Therapist program
• Licensed by the College of Occupational Therapists of Ontario
• 3+ years of experience preferred
• Strong clinical skills in treatment planning
• Valid Ontario driver’s license required
