Key Responsibilities:
• Assess psychosocial needs of patients and families
• Plan and implement effective discharge strategies

Requirements:
• Ontario College of Social Workers registration required
• Master’s degree in Social Work from an accredited university
• Preferred experience: 2-3 years in a hospital setting
• Solid interpersonal and coaching abilities
• Understanding of consent and capacity legislation
