Join Mackenzie Health as a Social Worker MSW, focusing on the Chronic Kidney Disease Program. Provide expert psychosocial assessments, counseling, and discharge planning to enhance patient care.

This role requires collaboration with an interdisciplinary team to support patients’ psychosocial needs throughout their care journey. You will conduct comprehensive assessments and facilitate community resource access, ensuring patients and families receive necessary support during their treatment. Your advocacy will help improve patient outcomes and care quality.

Key Responsibilities:
• Assess psychosocial needs of patients and families
• Plan and implement effective discharge strategies
• Consult on care planning with the healthcare team




• Identify and connect families with community resources
• Train healthcare staff on complex psychosocial needs

Requirements:
• Ontario College of Social Workers registration required
• Master’s degree in Social Work from an accredited university
• Preferred experience: 2-3 years in a hospital setting
• Solid interpersonal and coaching abilities
• Understanding of consent and capacity legislation
