In this full time position, you’ll be integral to an interdisciplinary team, delivering client-centered care and education. With at least five years of nursing experience, your role will prioritize effective communication, critical thinking, and cultural awareness to enhance client health outcomes.
Key Responsibilities:
• Deliver individualized health education and care
• Facilitate client advocacy and service referrals
• Collaborate within an interdisciplinary team setting
• Monitor and manage chronic disease conditions
• Participate in community outreach initiatives
Requirements:
• Graduate of a recognized Registered Nursing program
• 5 years’ experience as a Registered Nurse required
• 3+ years in a community health setting preferred
• Current Basic Rescuer CPR Certification needed
• Excellent verbal and written communication skills
