As a Nurse Navigator, you will play a vital role in the NTC Nursing Department by advocating for patients and facilitating their journey through the healthcare system. Your expertise in culturally relevant discharge planning and community engagement will enhance the health outcomes for Nuu-chah-nulth families. This position requires travel to diverse territories, enhancing service delivery across Vancouver Island.
Key Responsibilities:
• Lead patients through the hospital care system
• Build and maintain client advocacy processes
• Coordinate culturally safe discharge services
• Collaborate with Aboriginal Liaison Nurses in supporting health
• Assist members in assessing and addressing health needs
Requirements:
• BCCNM licensed as a Licensed Practical Nurse
• Minimum of 5-7 years in healthcare or patient education
• Familiarity with First Nation cultural factors
• Robust verbal and written communication skills
• Must have BC Driver’s license and travel capability
