Clinton Regional Health & Wellness Center has an exciting opportunity for a relief full‑time Primary Care Registered Nurse to join our team at Interior Health. This is a relief full‑time position until December 30, 2027, or return of incumbent. We are looking for a confident, self‑directed qualified who thrives in an autonomous role and is comfortable working independently while delivering high‑quality patient‑centered care. This day shift position operates Monday to Friday with shifts scheduled from 08:30 to 16:30.

How Will You Make an Impact?

As the Primary Health Care Nurse, you are responsible for providing comprehensive and coordinated nursing care to clients in a primary health care, community or patient home setting through assessment, planning,



implementation, and evaluation of care delivery. The Primary Health Care Nurse assists individuals to achieve secure, realistic and reasonable health‑care goals and to maintain optimal health through the provision of information/health promotion, disease prevention and treatment/care management services.

What Will You Work On?

The Primary Health Care Nurse is responsible for working in collaboration with an inter‑skilled care team and defined network of Primary Care Physicians/Nurse Practitioners. The goal is to conduct integrated care planning, offer client assessments/clinic visits, and provide ongoing monitoring and self‑management support follow‑up for clients (adults, seniors, pediatrics) and/or those with multiple chronic conditions, either in an individual or group setting and often directly within the family physician/nurse practitioner office.
