As a Registered Nurse (RN) in the Southwest Home Care program, you will provide essential nursing care and case management in the Edmonton zone. Your role involves assessing patient needs, offering therapeutic services, and developing client-centered care plans. You will work collaboratively with healthcare professionals and families to ensure optimal outcomes across various living environments.
Key Responsibilities:
• Assess client needs and establish care plans
• Deliver therapeutic nursing services in clients' homes
• Coordinate resources and evaluate patient's progress
• Provide case management and direct client care
• Mentor students during clinical experiences
Requirements:
• Graduation from an accredited nursing program
• Active registration with College of Registered Nurses of Alberta
• Two years of recent nursing experience
• Certification in Basic Cardiac Life Support
• Independent work ability in diverse home settings
