Description
This job posting is for a permanent part‑time position (not locum) to augment a family practice with a long‑term care (LTC) panel to provide challenging, diverse, and impactful work caring for the frail elderly.

Responsibilities and Expectations

Serve as a practitioner at Sunrise of Killarney, a 105‑bed long‑term care home.

Assist with resident caseload and support the current LTCI practitioners.

Participate in LTCI incentive payments, mentorship and care home onboarding, LTC‑specific events and projects, and the after‑hours care program.

Commit to formal LTCI best practice expectations:



regular proactive visits; on‑site medication reviews; attendance at care conferences; involvement in a 24/7 on‑call rota; completed documentation in charts.

Collaborate with an interdisciplinary team (physicians, nurse practitioners, nurses, allied health, and care home leadership) to provide holistic resident‑centered care.

Develop skills and confidence in long‑term care through a supportive community of practice.
