Job Description
The PACT Team (Police & Crisis Team) is a collaborative partnership between Mental Health & Addictions Services and Municipal Police Services/RCMP. Goals of the project are to enhance public safety and care for individuals, families and households with mental health issues through effective assessment, triage and intervention beginning at the point of contact in the community. The Assessor Coordinator Degree Senior provides screening, triage, assessment, treatment and case coordination services to an adult community mental health and/or forensic population. The incumbent provides program consultation/co-ordination, evaluation of clinical interventions, supervision, preceptor students in formal clinical practicum, and provides professional/public education regarding mental health issues and resources. Acts as a resource for other professionals for mental health in the community. Preference for this position will be given to HSAS membersin accordance with the terms and conditions of the SAHO/HSAS Collective Agreement

Experience
- Two (2) years related experience
- Experience working with mental health and addiction community outreach
Education
- Recognized degree in nursing , or a social work degree.
- Ability to be self motivated
- Knowledge in community resources and services in the clinical area
- Ability to work flexible schedule
- Knowledge of the Mental Health Services Act
- Knowledge in best practices in addictions and harm reduction strategies
- Ability to work as a member of a multi-disciplinary team
- Ability to work independently
- Exp. in mental health & addictions community outreach & crisis interventions
- Ability to respond under pressure.
- Knowledge in Electronic Medical Record systems
- Knowledge of crisis intervention and prevention programs
- Ability to enter and retrieve data in computer database systems
- Ability to build and maintain effective relationships/partnerships
- Ability to use a variety of interventions/programs
