As a Community Health Nurse, your responsibilities center around educating clients, reducing disease incidence, and acting within a Primary Health care model to enhance client capabilities in managing health challenges.

Key Responsibilities:
• Educate clients on health promotion and disease prevention
• Collaborate with the health care team for client support
• Facilitate access to comprehensive health services

• Guide clients through health-related life transitions
