Reporting directly to the Director of Health and Wellness, you will actively manage care assessments and document all activities in line with Extendicare's policies. Your involvement will extend to facilitating communications necessary for resident transitions and collaborating with the entire healthcare team to deliver optimal service.
Key Responsibilities:
- Complete resident assessments and ensure service plan accuracy
- Assist in organizing and contributing to care conferences
- Administer medications safely while adhering to regulations
- Educate residents and their families effectively
- Oversee care provider responsibilities and tasks
Requirements:
- Licensed Registered Nurse in your Province/State
- Experience with geriatric care preferred
- Strong oral and written communication skills
- Familiarity with Microsoft Office tools
- Recent Vulnerable Position Screening within 6 months
