FDuties & Responsibilities:

Responsibilities to Clients

· Conduct assessments to determine eligibility and need for service

· Refer clients for completion of RAI-CHA assessments as required

· Develop a plan of care for clients in consultation with team members

· Prepare clients and/or families for discharge

· Assist clients with recreational activities and personal care during the Day Away program and Overnight Respite Programs as required

· Act as a resource for other staff members

· Provide and/or delegate nursing care to clients attended Day Away/Overnight Respite

· Teach, delegate, assign, and supervise medication administration and other controlled acts permitted under the Regulated Health Professions Act, 1991

· Provide nursing care during overnight and daytime hours

· Provide assistance with activities for the clients attending programs

Responsibilities to Family/Caregiver Support

· Provide education, information, and counselling support to the caregivers of clients

· Ensure ongoing communication and support with families/caregivers of clients

· Provide information to family/caregivers regarding other support services in their community

Responsibilities to Volunteers/Students

· Assist in the recruitment of volunteers in conjunction with the Community Development department

· Assist in the orientation of volunteers and students to the program and support them as they carry out their day-to-day assignments

· Assist in the training of volunteers and students

· Ensure supervision and completion of evaluation of students/special placements





Responsibilities to the Team

· Be respectful and show a professional and cooperative spirit with all members of the multi-disciplinary team

· Participate in morning/afternoon huddles of staff and volunteers

· Participate in team meetings

· Participate with all team members in assessing client’s needs and evaluating planned interventions/programs

· Lead discussions surrounding client admissions and discharges

· Assist with orientation and training of new team members

Responsibilities to Administration

· Completion of admission, assessment, and referral forms as per policy and procedure

· Ensure client records are maintained and current

· Gather statistics as required by Day Away Manager

General Duties

· Assist in the promotion of the services of HCSS, especially to healthcare professionals

· Seek ongoing education opportunities which will enhance knowledge pertinent to the role, including about HCSS Grey-Bruce services, clients, and the sector

· Maintain privacy and confidentiality

· Adhere to the Occupational Health and Safety Act and its associated regulations

· Complete mandatory annual online training courses

Competencies & Qualifications

· Must be available for overnights/evenings

· Excellent communication and problem-solving skills

· Above-average competence with current technology and software

· Solid interpersonal skills and ability to work effectively as a member of a team

· 3-5 years’ experience working with the elderly and the physically challenged and their families.

· Must have a valid Driver’s License and access to a reliable vehicle

· Knowledge of Alzheimer's Disease and related disorders

· Previous experience working with volunteers is an asset

· Current registration with the College of Nurses is required

Compensation & Total Rewards

· Wages are from $27.22/hour

· 10 days of PTO per annum

· 3 weeks of vacation, per annum

· Benefits, including dental

· TFSA/RRSP matching program, up to 5%
