Job Overview

We are seeking a dedicated Family Practice Provider to join our healthcare team, offering an exceptional opportunity to serve a large patient population of approximately 900 individuals. This role involves comprehensive primary care services within a dynamic clinical setting, focusing on delivering high-quality patient-centered care. The ideal candidate will have experience in outpatient settings, with a strong background in patient care and medical documentation.

Key Responsibilities

- Provide comprehensive primary care for acute and chronic medical conditions.
- Conduct patient assessments, physical examinations, diagnostic investigations, and ongoing follow-up.
- Develop and manage treatment plans, prescriptions, referrals, and preventive care.
- Provide pediatric care, routine health screening, and patient education.
- Offer home-care services to eligible patients, where appropriate.
- Maintain accurate and timely documentation using OSCAR Pro EMR by WELL Health.
- Collaborate effectively with physicians, nurses, administrative staff, and other healthcare professionals.

Practice Opportunity and Overhead

- Approximately 900 patients are available to be rostered to the successful physician, subject to patient choice and applicable professional requirements.
- The first six months will be a mutual probationary period, during which the physician will practise on a fee-for-service basis.
- Following successful completion of the probationary period, the transfer and rostering of patients under the physician’s name may begin.
- Clinic overhead is initially 25%.




- The overhead rate will be reduced to 20% once the physician’s roster reaches 1,500 patients or more.
- The clinic will provide administrative and operational support to assist the physician in establishing and growing their practice.

Qualifications

- Medical degree and eligibility to practise family medicine in Ontario.
- Current and unrestricted registration with the College of Physicians and Surgeons of Ontario (CPSO).
- Valid CMPA coverage.
- Primary care or family medicine experience is preferred.
- Experience in acute care, pediatrics, chronic pain management, or home care is considered an asset.
- Familiarity with OSCAR Pro or another electronic medical record system is preferred.
- Experience in clinical research, quality improvement, or data collection is an asset.
- Solid communication, organizational, and patient-care skills.
- Compassionate and professional approach to diverse patient populations.
- Ability to work effectively within a multidisciplinary healthcare team.
