Your Opportunity:
The East Calgary Family Care Clinic is known for its innovative approach to providing comprehensive primary care for people with complex needs. This could be any combination of chronic illnesses, mental health difficulties and addiction along with social, economic or cultural factors that negatively impact health. Reporting to the management of the East Calgary Family Care Clinic, the LPN works within their scope of practice utilizing the nursing process, in collaboration with an interdisciplinary team. Primary responsibilities will include triage including telephone triage and intake appointments for new patients to the clinic, as well as procedures such as routine wound care, 24-hour BP monitoring, assess and manage ear care (ear flushes), vaccinations, screening tests, wound care, other injections as ordered, and phlebotomy. This role also has a focus of care coordination of patients with complex needs using a case management approach through development of therapeutic relationships. Additional responsibilities include guidance to unregulated healthcare workers. In this role, the LPN works both independently and part of an interdisciplinary team which requires a strong ability for critical thinking around the triage of urgent phone calls of complex clients in possible crisis situations.

As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team utilizing nursing processes, critical thinking, problem solving and decision making skills. You will be guided by your education, experience, demonstrated skill level and PCA policies and procedures. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of PCA.

Required Qualifications:
Completion of an accredited practical nurse education program. Active or eligible for registration and practice permit with the College of LPNs and HCAs of Alberta (CLHA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional Required Qualifications:
2 years LPN experience within the last 5 years. Ability to conduct contemporaneous electronic documentation during phone calls or patient visits with a 45-wpm rate. Experience with an EMR system and demonstrated aptitude with computers and technology. Demonstrated attention to detail and accuracy. Experience working with patients with complex needs Assessment of skills to recognize and respond appropriately to changes in patients’ status Recent skills with wound care assessment and management Ability to be self-motivated and work independently Solid communication skills utilizing a variety of modalities and ability to adapt communication style to meet the needs of a specific patient; relay patient information to other health care providers in a timely, concise, accurate manner in order to facilitate the primary health care providers’ treatment decisions Balance numerous demands and conflicting priorities Work within a team environment and effectively contribute to patient care planning
Preferred Qualifications:
Ambulatory and outpatient experience specifically primary care. Experience working on a medical unit/internal medicine. Experience in patient triage including phone triage. Experience in providing diabetic foot exams. Experience in working with patients with addictions and/or mental health issues. Experience in case management.
