Duties

- Deliver personalized patient care, including assistance with activities of daily living such as bathing, dressing, grooming, and mobility support
- Administer medications and monitor health conditions in accordance with care plans and protocols
- Assist with meal preparation and feeding to ensure nutritional needs are met
- Support clients with dementia or cognitive impairments through engaging activities and supervision
- Perform light housekeeping tasks like cleaning, laundry, and maintaining a safe environment
- Provide emotional support and companionship to promote mental well-being
- Collaborate with healthcare teams to document care activities, report changes in condition, and ensure seamless communication





Requirements

- Previous experience working in assisted living facilities, nursing homes, or home care environments is highly valued
- Solid background in patient care, senior care, hospice care or caregiving roles involving individuals with disabilities or dementia
- Ability to perform heavy lifting safely when assisting clients with mobility or transfers
- Knowledge of medication administration procedures and safety protocols
- Excellent communication skills and a compassionate approach to caregiving
- Physical stamina to handle demanding tasks such as cleaning and assisting with mobility needs
- Certification as a Personal Support Worker (PSW), Health Support Worker (HSW), or equivalent is preferred but not mandatory; relevant experience can substitute
- Valid driver’s license or reliable transportation is an asset for home visits
