64 bed LTC Home and 24 unit Retirement Home in Alliston, Ontario is looking for permanent part time RPN's; permanent part-time day and permanent part-time evening lines available including every other weekend; scheduling by availability for shifts (days/evenings/nights) also available.
Accountability: Responsible to the Administrator/DON, and/or Director of Care/IPAC.
Qualifications:
- Current Certificate of Competence as a Registered Practical Nurse issued by the College of Nurses of Ontario required;
- Experience in Long Term Care or Retirement as a Registered Practical Nurse preferred;
- Nursing leadership experience preferred;
- Demonstrated ability to foster and maintain excellent working relationships with a broad range of individual and partner healthcare organizations;
- Demonstrated ability to engage and collaborate with Residents and families to achieve best possible outcomes;
- Demonstrated leadership skills as an effective member of the multidisciplinary team, coupled with the ability to work independently;
- Proven success in the areas of interpersonal relationships and mentoring/teaching;
- Excellent verbal and written communication skills;
- Self-directed, organized, and dependable;
- Consistent and reliable performance record including attendance;
- Proficient in computer programs including Microsoft Word/Excel/Power Point/Outlook.
Key Responsibilities:
- Applies the nursing process by assessing, planning, implementing,
teaching and evaluating nursing care for Residents, families or groups;
- Assesses needs of Residents and organizes Resident care to meet the total needs of the Resident on the shift;
- Assists in developing and modifying of Resident Care Plans based on assessed needs and orders of, including but not limited to, a physician, nurse practitioner, dietitian;
- Participates with other nursing staff in the evaluation of Resident care to ensure that care is being given as directed on the Resident Care Plan;
- Participates in the development of assignments for PSW duties on shift accordingly, to provide effective Resident care, to assure continuity of care with consideration given to the physical, spiritual, social, emotional and rehabilitative needs of the Resident;
- Directs and supervises the nursing care given by PSW's;
- Provides medication and treatment administration to assigned Residents;
- Participates in weekly Physician rounds, including Resident focused discussions, facilitating assessments, processing of physician orders, scheduling of diagnostic testing and consults, documentation and notification of appropriate parties;
- Mentors staff to maintain positive relationships by demonstrating respectful interactions and affirming successful actions performed by staff;
