Job Overview

We are seeking a dedicated and compassionate Nurse Practitioner to join our healthcare team, providing high-quality patient care across diverse settings. In this vital role, you will serve as a primary care provider, collaborating with physicians, nurses, and other healthcare professionals to deliver comprehensive medical services. Your expertise will help improve patient outcomes, especially within senior care, pediatric populations, and acute care environments. Duties

- Conduct thorough patient assessments, including history-taking and physical examinations for diverse age groups, with a focus on pediatrics and senior populations.
- Diagnose illnesses, develop treatment plans, and prescribe medications in accordance with state regulations and medical protocols.
- Manage chronic conditions such as dementia, pain, and other complex health issues through medication management and case coordination.
- Provide patient education on health maintenance, disease prevention, medication use, and lifestyle modifications.
- Collaborate with multidisciplinary teams to coordinate care in settings such as nursing homes, assisted living facilities, hospitals, or home care environments.





Qualifications

- Valid Nurse Practitioner license with current certification to practice independently.
- Experience working with children (pediatrics), seniors (senior care), or in acute care settings.
- Prior nursing home or assisted living experience highly preferred.
- Strong knowledge of medical terminology, medication administration protocols, and case management principles.
- Hands-on experience with electronic health records such as Epic or Cerner is desirable.
- Skilled in patient care techniques including pain management and dementia care.
- Excellent communication skills to effectively educate patients and collaborate with healthcare teams.
- Ability to perform laboratory procedures such as phlebotomy and interpret laboratory results accurately.
