Position Overview
Purple Pharmacy is seeking a compassionate, reliable and experienced Health Care Assistant to support our community-based Medication Administration Program (MAP).
The successful candidate will visit eligible patients in their homes and provide direct assistance with medication administration according to each patient’s medication administration record, care plan, pharmacy procedures and the requirements of the BC PharmaCare Medication Administration Program.
This position is ideal for a registered Health Care Assistant who enjoys working independently in the community while remaining closely connected with pharmacists and other health care professionals.
Responsibilities
- Travel to patients’ homes according to the assigned daily schedule
- Provide direct assistance with medication administration within the Health Care Assistant’s authorized training and scope
- Administer or assist patients with medications according to the medication administration record and pharmacy-approved care plan
- Confirm the correct patient, medication, dose, route, time and instructions before providing assistance
- Observe patients before, during and after medication administration
- Document every patient visit and medication administration accurately and promptly
- Record refused, missed, delayed, unavailable or unsuccessful doses
- Report medication discrepancies, side effects, changes in condition and patient concerns immediately to the supervising pharmacist
- Follow pharmacy procedures for medication errors, adverse reactions, falls, emergencies and other reportable incidents
- Communicate professionally with patients, caregivers, pharmacists, nurses, prescribers and other members of the patient’s care team
- Maintain accurate medication administration records,
visit logs and other required MAP documentation
- Confirm medication supplies and notify the pharmacy when refills, packaging changes or delivery coordination may be required
- Support medication delivery and secure transfer of medications when assigned
- Maintain patient privacy, dignity, independence and confidentiality
- Follow infection-prevention, hand-hygiene and workplace-safety procedures
- Work independently while remaining available by telephone to the supervising pharmacist
- Assist with general pharmacy or administrative duties when scheduled to work inside the pharmacy
- Perform other related duties consistent with the employee’s qualifications and authorized scope
Required Qualifications
- Completion of a recognized Health Care Assistant, Health Care Aide, Community Health Worker or equivalent program
- Current registration in valuable standing with the BC Care Aide & Community Health Worker Registry
- Knowledge and demonstrated competency in safe medication assistance and administration
- Current First Aid and CPR certification
- Valid Class 5 BC driver’s licence
- Reliable personal vehicle and ability to travel between patients’ homes
- Valid vehicle insurance appropriate for work-related travel
- Ability to successfully complete a criminal record check, including vulnerable-sector screening where required
- Strong English communication and documentation skills
- Ability to use a mobile phone,
electronic documentation system and basic pharmacy software
- Ability to work independently, follow detailed instructions and manage a time-sensitive visit schedule
- Demonstrated reliability, professionalism, compassion and attention to detail
Preferred Qualifications
- Previous experience providing medication assistance or administration in home care, assisted living, community pharmacy or a similar setting
- Previous experience working with seniors, patients with disabilities or patients with complex medication needs
- Experience with blister packs, compliance packaging or medication administration records
- Training in dementia care, mental-health support, diabetes care or community support services
- Additional language skills are considered an asset
- Previous experience working with a pharmacy-led medication administration program is an asset

- Physical ability to travel between residences and safely perform assigned duties
