: The overall function of the Assessor/Coordinator (Client Care Coordinator) position is to provide case management services which includes assessment, planning, monitoring and evaluation. The Client/Patient Access Services is primarily responsible for a single entry access and ongoing coordinator for a variety of community based services within Saskatoon Health Region.

Education
- Bachelor of Science in Nursing (BScN)
Other Education and Training
- Bachelor of Occupational Therapy is acceptable
- Bachelor of Social Work is acceptable
- Bachelor of Physical Therapy is acceptable
Knowledge and Abilities
- Knowledge in the SHA, community resources and applicable programming
