- Graduate of an approved School of Nursing and currently registered with the CNO; B.Sc.N preferred

- Ability to attend work regularly; demonstrated by an acceptable attendance record The Case Manager is responsible for managing a caseload of patients; creating a milieu for empowerment in the provision of patient focused care; promoting collaborative practice, coordination of care and continuity of patient care among team members; and integrates the components of evidence based practice to improve the quality of care provided to patients.

The Medically Complex
Program provides care to patients with a broad range of complex medical conditions, including those with advanced neurological conditions and chronic illnesses. These individuals require ongoing medical care, skilled nursing care, and/or assessment and active management by specialized interprofessional teams.
