- Provide comprehensive, patient-centred primary care across the lifespan, including assessment, diagnosis, treatment, and ongoing management of acute, chronic, and complex health conditions.
- Develop and implement individualized care plans in collaboration with patients, families, and the interprofessional team to optimize health outcomes.
- Deliver preventive care and health promotion services, including routine screening, immunizations, well-woman care, well-baby and well-child care, prenatal care, and patient education.
- Conduct medication reviews, monitor investigations, and support chronic disease management, self-management, and health literacy.
- Assess and manage urgent and episodic health concerns, providing timely intervention, triage, referral, and follow-up within the Nurse Practitioner scope of practice.
- Coordinate care for patients with complex medical and social needs by collaborating with physicians, specialists, allied health professionals,



community agencies, and other healthcare providers to ensure seamless transitions and continuity of care.
- Facilitate access to community resources and support services that address health and social determinants of health.
- Maintain accurate, timely, and comprehensive clinical documentation in accordance with professional standards and organizational policies.
- Participate in quality improvement initiatives, program evaluation, clinical protocol development, and evidence-informed practice to enhance patient care and organizational performance.
- Contribute to teaching, mentorship, and the orientation of students, learners, and new team members while supporting a collaborative, interprofessional practice setting.

Pay: $55.00-$65.00 per hour
