Job Description

We are seeking RPNs to join our team to support the new Peritoneal Dialysis Assisted Program. Reporting to the Lighthouse and Nursing Leadership, the Registered Practical Nurse will provide excellent peritoneal dialysis support to individual clients in the comfort of their homes. Applicants are expected to possess the clinical nursing skills to perform as required, supplemented by further training in peritoneal dialysis support.

Duties and Responsibilities:

- Clinical monitoring
- Volume assessment; weight gain/loss based on target weight, edema, BP, PD and fluid intake and output
- Cardiovascular and respiratory assessment
- Vital signs monitoring and blood glucose monitoring as required
- Wound assessment
- Peritoneal Dialysis(PD) catheter insertion site assessment, care and dressing change
- Administering intraperitoneal antibiotics - oral, subcutaneous and intravenous - while monitoring medication compliance
- Adhering to injection control requirements consistent with evidence-based practice for PD care
- Providing education on disease-specific pathways; patient safety and so on
- Documentation of clinical notes as per College of Nurses of Ontario (CNO) guidelines and Lighthouse Retreat policies and procedures
- Adherence to the applicable policies, procedures and guidelines outlined by the Lighthouse Retreat and its partner hospital.
- Responding to resident/family concerns and ensuring appropriate action is taken and or concern communicated to Nursing Leadership and other departments




- Upholding and promoting the organization’s values and philosophy relating particularly to ethics, morality, and integrity as set out in the Company’s Code of Conduct
- Completing all other tasks as assigned

Qualifications:

- Registration with the College of Nurses of Ontario as a Registered Practical Nurse, in good standing

Minimum one year of experience (preferred)

- Demonstrated knowledge of and compliance with safe work practices
- Must possess strong written and oral communication, critical thinking, care coordination, time management and customer focus skills
- Demonstrated empathy, concern and interest in providing quality care and service for residents
- Physical ability and availability to perform the essential duties of the job in all shifts
- Must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past six (6) months

- Demonstrated understanding of and commitment to The Lighthouse Retreat Inc.'s comprehensive safety programs and practices including staff, patient and environmental safety and participation in regular in-services in this area

Job Type: Part-time

Salary: $28 per hour

Schedule: Day, evening and weekend availability required
