For information purposes only, please note that this position currently oversees a 24-bed Transitional Care unit, a site-specific Recreation Therapy team, and a portion of the Mobile Team.

Transitional care provides safe, short stay care for medically secure patients while they prepare for discharge to the community or another care setting. Our team works closely with patients to provide structured routines and therapy sessions to maintain strength and function, meet care needs, and prepare patients for discharge.

The Mobile

Team includes Personal Care Attendants, Registered Practical Nurses, Registered Nurses and Allied Health Professionals (Physiotherapist, Occupational Therapist, Social Worker, Speech Language Pathologist and Dietitian). Some of the team members are site specific and others work across both Bruyère Health Centretown and Market sites (including Hospital and Long Term Care programs). This role currently works with the Registered Nurses and Allied Health mobile team members.

The Clinical

Manager reports directly to the Director, Clinical Programs, Transitional Care, with cross reporting to the Director, Stroke Rehabilitation, Allied Health Professional Practice and Therapeutic Recreation. This role involves working very closely with leaders across both sites, including but not limited to,



patient relations, patient flow, unit coverage and matters related to clinical operations. Defined benefit pension plan (when applicable) Wellness program and on-site gyms Mental health support through our Employee and Family Assistance Program Professional development and learning opportunities Reporting to the Director, Clinical programs, you will: Plan, organize, direct, control and evaluate all activities of the assigned unit in order to provide high quality patient care.

Manage the administrative and clinical of a patient care unit in accordance with the Public Hospitals Act, the philosophy, mission and strategic direction of Bruyère Health, the standards from Accreditation Canada, the Regulated Health Professionals’ Act, the Occupational Health and Safety Act and the relevant collective agreements. Manage clinical issues, practices and home first philosophy. Undergraduate degree in Nursing or in an Allied Health Profession.

Minimum of two (2) years’ experience in Medical, Surgical Nursing or related Allied Health. Master’s degree in nursing, Health, Public, Education or Business administration. Fluent in French and English.
