Make a difference in seniors' lives as a Geriatric Occupational Therapist with Senior Persons Living Connected. This full time, on-site role focuses on assessments and interventions for older adults. As part of the GAIN Community Team, the Occupational Therapist will utilize standardized assessment tools to support seniors facing emotional, psychological, and physical challenges of aging.
You will collaborate with an inter-professional team, contribute to care plans, and enhance daily living activities for frail older adults. A focus on meaningful interventions ensures impactful patient care. Key Responsibilities:
- Conduct comprehensive geriatric assessments collaboratively
- Complete clinical triage for new patient referrals
- Gather and record data across assessment domains
- Provide clinical care to enhance daily living activities
- Analyze seniors’ capabilities through observation and assessment Requirements:
- University degree in Occupational Therapy
- Registered with the College of Occupational Therapists of Ontario
- 2-3 years of relevant experience
- Knowledge of dementia and behavioral conditions
