Strides Toronto is seeking a full time Urban Tele-Mental Health Coordinator to enhance access to mental health services with a hybrid work model. This role focuses on partnership building, community outreach, and facilitating psychiatric consultations. In this permanent position, you will coordinate referrals and support case workers while closely liaising with The Hospital for Sick Children.
Your administrative skills will ensure all records are meticulously maintained, and your outreach initiatives will promote the Tele-Mental Health Program. Effectively engaging community partners will be crucial to improving specialized mental health care access. Key Responsibilities:
- Coordinate referrals and confirm psychiatric appointments
- Document all referral statuses in the database
- Develop outreach plans to engage community services
- Foster partnerships with healthcare providers
- Support program activities and maintain accurate documentation Requirements:
- Post-Secondary Education in Human Services or related field
- Minimum 2 years of administrative experience
- Knowledge of CYMH and UTMH Services
- Intermediate proficiency in Microsoft Office
