Maamwesying North Shore Community Health Services Inc. is seeking an Indigenous System Navigator for the Emergency Department to bridge hospital care with Indigenous communities, providing advocacy, system navigation and care coordination in a culturally secure manner.
The ISN supports timely access to services, discharge planning, and continuity of care, working with ED clinicians and community partners to improve health outcomes. Ability to travel and maintain regulatory memberships is required.
