Vancouver Coastal Health (VCH) is proud to be part of the provincial implementation of minimum Nurse to Patient Ratios (mNPRs), an initiative aimed at enhancing nursing practice and strengthening quality of care across the health system. mNPRs set the minimum number of nurses providing care to patients on a given unit. In British Columbia, mNPRs are developed in partnership with the Ministry of Health, the BC Nurses’ Union (BCNU) and health organizations. Description

JOB SUMMARY:

Within a person and family-centred care framework and a continuum-based model of service delivery, and in alignment with professional standards and organizational vision, values, and policies, the Indigenous Intake and Transition Support Nurse (Nurse) provides health system navigation for Indigenous patients across acute and community health contexts, inclusive of chronic, palliative, rehabilitative, and/or mental health and substance use services across VCH’s care regions. Within a regional portfolio, the Nurse will assess, plan, coordinate, implement, and evaluate care for Indigenous patients using practices that include case management, health education, complex health system navigation and patient and family support. They are responsible and accountable for identifying the need for and developing/or partnering on procedures, workflows, decision support tools and education that facilitate complex intakes and safe transitions across VCH health systems.

The Nurse leads clinical discussions to support safe patient intake and transition with care teams and other health care professionals and advises on the availability and eligibility for community and acute services.

QUALIFICATIONS:

Education & Experience

Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Five (5) to seven (7)



years recent related experience in the designated clinical program area or an equivalent combination of education, training and experience.

Pursuant to Section 41 of the British Columbia Human Rights Code, preference will be given to individuals who identify as Indigenous (First Nations, Inuit, M�tis).

Knowledge, Skills & Abilities

- Broad knowledge of nursing theory and practice within a patient and family centered care model.
- Broad knowledge of the British Columbia College of Nurses and Midwives (BCCNM) standards of practice for nursing.
- Demonstrated ability to provide person-centred care to support health and wellness.
- General knowledge of Trauma and Violence Informed Practice and Harm Reduction approaches and Strengths-based care.
- Demonstrated ability to promote Indigenous Cultural Safety.
- Demonstrated ability to provide competent and culturally secure care in a variety of settings and with diverse populations.
- Demonstrated understanding and in-depth knowledge of First Nations, Inuit, and M�tis Peoples and history.
- Demonstrated understanding of provincial and federal guiding principles and documents, for example In Plain Sight Report, and Calls to Action, to eliminate Indigenous-specific racism in the health care sector.
- Demonstrated ability to identify and recognize current health care system gaps and barriers for Indigenous individuals and communities seeking health care through the responsible collection, analysis and use of Indigenous health data.
- Demonstrated ability to identify and recognize power inequities within the healthcare system associated with ongoing impacts of colonialism.
- Demonstrated ability to assess patients at risk and knowledge of appropriate acts, e.g., Mental Health Act, Adult Guardianship Act, Public Health Act, Health Care (Consent) and Care Facility (Admission) Act as it relates to the rights and obligations of patients and staff.




- Knowledge of clinical practice related to treatment and care of the designated patient population.

- Demonstrated computer literacy to operate a computerized patient care information system and word processing, spreadsheet, Internet and e-mail software.
- Physical ability to perform the duties of the position.
