Place seeks an enthusiastic Housing Support Worker to participate in the mission of HART Hub Dufferin. This role emphasizes providing Housing First focused support, engaging in case management, and connecting clients with housing resources. You will also collaborate with internal and external partners to enhance the quality of life for individuals facing homelessness and mental health challenges. Key Responsibilities:

- Clarify Housing Support Workers' role to clients
- Perform comprehensive SPDAT assessments promptly




- Schedule regular check-ins with clients for ongoing support
- Advocate for housing solutions and teach necessary life skills
- Facilitate referrals to local community resources Requirements:
- Degree in Social Work, Social Services, or similar
- At least 2 years’ experience with homeless populations
- Solid understanding of Housing First Principles
- Strong communication and interpersonal skills
