In this energetic role, you will conduct mental status assessments, develop tailored care plans, and assist clients with daily activities. Collaborate with a multi-disciplinary team while connecting clients to valuable community resources and health education. Key Responsibilities:

- Conduct comprehensive mental health assessments
- Create and evaluate client-centered care plans
- Assist clients with daily living activities
- Provide education and connect clients to resources
- Maintain accurate client records and reports Requirements:
- Minimum 6 months mental health nursing experience
- Valid driver’s license and vehicle
- Completion of a Community Mental Health Course
- Current College of Nurses of Ontario registration
