This position is part of the Long Term Care Initiative, allowing practitioners to play a vital role in improving resident health. You will collaborate with an interdisciplinary team to provide patient-centered care and support the development of your skills in long-term care through mentorship and community engagement.
Key Responsibilities:
• Provide holistic, resident-centered care for elderly patients
• Conduct regular proactive visits and in office medication reviews




• Attend care conferences and participate in after-hours care
• Ensure thorough documentation in patient charts
• Collaborate with an interdisciplinary health care team
Requirements:
• Willingness to obtain Vancouver Coastal Health privileges
• Experience in long-term care is preferred
• Commitment to the Long Term Care Initiative
• Ability to participate in on-call responsibilities
• Familiarity with PointClickCare EMR system
