Join VHA Home HealthCare as a Registered Nurse in West Toronto, providing specialized in-home care for adults. In this role, you will support the Adult Visiting Nurse program with evening shifts on weekends.

Ideal candidates will deliver high-quality, client-centered care to individuals aged 18 and over in their homes. You will collaborate with other healthcare professionals, while forming meaningful relationships with clients and their families. Key Responsibilities:

- Assess and coordinate comprehensive care for clients
- Monitor and evaluate ongoing care plan effectiveness
- Ensure patient safety and quality in care delivery
- Maintain accurate documentation per CNO standards
- Participate in team meetings and on-call rotations Requirements:
- Valid RN or RPN with valuable standing from CNO
- Ontario driver's license and reliable vehicle required
- CPR certification and wound care knowledge preferred
- Experience in Acute Care or Medical-Surgical nursing
