Your expertise in nursing will allow you to deliver comprehensive care and conduct joint care planning with interdisciplinary teams in a client-centered manner. - Provide comprehensive care within an interdisciplinary team
- Serve as primary contact for client’s care plans




- Conduct client assessments and develop tailored care plans
- Facilitate interdisciplinary communication and meetings
- Organize family and caregiver meetings during transitions Requirements:
- Registered with the College of Nurses of Ontario
- Two years of nursing experience required
- Experience in home/community care or acute care
- Excellent assessment and organizational skills
- Valid driver's license and ability to travel Utilize your nursing skills in a rewarding home care setting that emphasizes community support and client wellness.
