In this part-time role, you will conduct client assessments, develop individualized care plans, and support transitions in care. You are expected to work from Monday to Friday between 2 PM and 6 PM, providing essential support and engaging with community health services. Key Responsibilities:
- Conduct detailed client assessments and create care plans
- Act as a primary provider for assigned client caseloads

- Current Certificate of Competence with the College of Nurses of Ontario
